The Holmdel Police Department frequently receives inquiries from government, business, and citizens on the regulations and procedures for obtaining an individual's criminal history record.  The following New Jersey State Police web site will assist you in answering the most common questions asked about this process

Police Reports: Accident / Arrest / Investigation / Discovery

1st - 10th pages $0.75 per page
11th - 20th pages $0.50 per page
21st + pages $0.25 per page

(plus cost of postage)

Videotapes: $15.00     CDs:  $5.00    DVDs: $5.00

Audio Cassette Tape: $25.00

Photos:

3 x 5 $1.00 each
8 x 10 $7.00 each
11 x 14 $13.00 each

Firearms Fees:

INSTRUCTIONS FOR FIREARM APPLICATIONS

MUST BE A TAX PAYING RESIDENT OF HOLMDEL TOWNSHIP

 New applications (never had a New Jersey Firearm ID)

  • Certified check or money order in amount of $60.25 made payable to “New Jersey State Police S.B.I.” for the processing of your fingerprint forms
  • Form #STS-33 - Application for ID card and/or purchase permit – check box on top for ID and/or permits (how many) and fill in boxes 1-31
  • Form #SP-66 – Mental Health Records Search
  • Authorization and Release
  • $5.00 - Fee for ID card - cash or personal check payable to “Holmdel Township”

*Upon completion of paperwork please contact the Holmdel PD Records Bureau to arrange an appointment for fingerprinting. 

Handgun purchase permit (you must already have a Firearm ID card)

  • Certified check or money order for $18.00 made payable to “New Jersey State Police S.B.I.”
  • Form #STS-33 – Application for ID card and/or purchase permit – check box on top for purchase permit and number of handgun permits requested, fill in boxes 1-31
  • Form #SP-66 – Mental Health Records Search
  • Form #SBI-212A – Request for Criminal History Record
  • $2.00 – Fee for each Handgun Permit – cash or personal check payable to “Holmdel Township”

Change of address / lost card / mutilated card / change of name

  • Certified check or money order for $18.00 made payable to “New Jersey State Police S.B.I.”
  • Form #STS-3 – Application for Duplicate Card-check reason for new card-fill in boxes 1-26
  • Form #SP-66 – Mental Health Records Search
  • Form #SBI-212A – Request for Criminal History Record
  • $5.00 - Fee for ID card - cash or personal check payable to “Holmdel Township”
  • Old card (if applicable)

Signatures must be witnessed by Holmdel Township Police/Records Bureau.

Employer and references (not relatives) must have full name, street address, zip codes and telephone numbers.

Form numbers are located on bottom left corner of each form.
